The following are a number of checklists that readers might find useful in their practice setting. Check lists are generally provided in MS Word™ Format so that users can edit them to their individual requirements.

All files have been set to open in a secondary Window. However some files may not open depending on your browser.

Files may be printed or saved from this Window. Files may be adapted for use by the user.

Depending on your browser generally only PDF and MS Word files can be opened within a browser, although some users may also be able to open Excel files; all other file types must be downloaded or saved before opening.

We have also listed check lists available on the Web. We have not provided these on the CD because they change from time to time and the Web is therefore the best source for the up-to-date resources.  These links will also open in a new Window. ô MBS EPC Checklists on the Web

45 Year Old Health Check

From 1 November 2006, a new MBS item (717) will be available for GPs to provide a preventive health check for people between the ages of 45 and 49 (inclusive) who are at risk of developing a chronic disease

Comprehensive Medical Assessments for Residents of Aged Care Homes

A Medicare rebate for Comprehensive Medical Assessments was introduced in 2004 to support GPs in undertaking a full systems review of permanent residents of aged care homes.

Item 712 provides a Medicare rebate for a full systems review, including an assessment of the resident's health and physical and psychological function. It involves a GP taking a detailed relevant medical history, conducting a comprehensive medical examination, developing a list of diagnoses or problems, and providing a written summary of the outcomes of the CMA for the resident's records.

Health Assessments for people aged 75 + over (55 + over for Aboriginal and Torres Strait Islander people

Information on EPC older age health assessments, including item descriptors, explanatory notes, questions and answers (for providers and consumers), a case study and a checklist.

Annual, voluntary health assessments for older Australians provide an opportunity for a GP to undertake an in-depth assessment of the patient's health. Health assessments cover the patient's medical, physical, psychological and social function. The GP should also assess whether preventive health care and education should be offered to the patient, to improve that patient's health and physical, psychological and social function.
 

Multidisciplinary Case Conferencing

Information on EPC case conferencing, including item descriptors, explanatory notes, questions and answers (for providers and consumers), a case study and a checklist.

An EPC case conference is a meeting of health and care providers to plan for the health and care needs of an individual patient with at least one chronic medical condition and complex multidisciplinary care needs requiring care from a GP and at least two other health or care providers. Case conferences may be undertaken for patients in the community (community case conferences), patients being discharged into the community from hospital or day hospital facilties (discharge case conferences), or people living in Residential Aged Care Facilities (Residential Aged Care Facility case conferences).

Two Yearly Adult Aboriginal and Torres Strait Islander Health Check (for 15 to 54 year olds)

Information on the Two Yearly Health Check for Adult Aboriginal and Torres Strait Islander People introduced to the Medicare Benefits Schedule in May 2004.
