A key direction in the National Chronic Disease Strategy is to tailor self-management support to the needs of target population groups, particularly Indigenous peoples, the socio-economically disadvantaged, and young people.
The case studies in Section Three of this guide reported a range of CDSM support models developed to suit particular needs and contexts. The findings, barriers or facilitators relevant to a particular case study were included as ‘Lessons’ in those case studies.
While some of the lessons were specific to a particular case study, other more general learnings emerged.
