ô Introduction

In recognition of the need for health care providers to integrate their service delivery with a wide range of other providers and institutions the CDSM Support Project has developed a database to aid the management of provider contacts and referral pathways. The database is included on the CDROM and on the Web.

The Service Directory is a MS Access Database written by Catherine Leahy from the University of Adelaide and can be used to record the contact details of health service providers, health services and other agencies that might be useful for referring clients. The database provides searching, reporting, data export and database administration functionality and is based around contact with institutions, (e.g. hospitals, pathology laboratories or schools) or health care providers (e.g. GPs, allied health or specialists).

The database could also be used by individuals and practices as a general contacts database as well as specifically as a CDSM support tool practice or could be converted to a web-based product that could be maintained by a local Division of General Practice.

ô Downloading and Installation

The Service Directory Database can not be run from within the Browser Window but must be downloaded first. Also users should never use a short cut to open the Service Directory database. It must be run from within MS Access.

To download the file, Right Click on the file link below and Select "Save Target as .." from the context menu. Choose a location to save the file and then press OK.

Download: ServiceDirectoryDB.mdb

Then open MS Access and use File | Open from the menu to open the file ServiceDirectoryDB.mdb which is the main file for the Service directory. Depending on your security settings a number of error messages may appear; ignore these messages.

Passwords
The Service Directory password is 'sddata' (this is mentioned in the Service Directory manual). This password is intended to prevent unauthorised access to the Service Directory.

The Service/Contact Type Administration password is 'access1' (this is NOT included in the Service Directory manual). This password is provided to prevent unauthorised access to the administration area of the database where changes to Service provider and Service categories are made.

Activation codes
Note: The last page of the manual talks about entering activation codes.  The activation code ensures that the database will continue working after 2 months in the event that the Demo Version of the Database is being used. The Database version on this CD is already activated.
